Please Note: Virtual visits with Bennett Health and Wellness do not replace ongoing primary care. Patients are encouraged to maintain regular visits with their primary care provider for long-term health management.

Scheduling Your Appointment

We offer flexible telehealth appointments designed to fit your busy life. Visits can be booked online through our secure scheduling platform.

Once your appointment is confirmed, you’ll receive a short intake form to complete before your visit. This form includes:

  • Patient consents and pricing acknowledgment
  • Brief medical history
  • Preferred pharmacy information
  • Insurance details (for prescription verification only — all visits remain self-pay)

Completing your intake form early helps your visit run smoothly and allows prescriptions to be sent promptly when appropriate.

All visits are self-pay (cash-only). Payment is due at the time of service.

Bennett Health and Wellness operates on a simple, transparent self-pay model — no insurance required. Pricing is clearly communicated before your visit, and payment is due at the time of service.
